Sorry if this is late but the reason it's directed toward Tokiomi is because he is a master and Gilgamesh convinced Kirei that to find his answer he would need the grail and that if he followed that path he would need a servant strong enough to kill Gilgamesh to which Kirei decides to murder Tokiomi to gain Gilgamesh as his servant. so yes there was strategy behind it and they were perfectly positioned for it.
Sadism was a certainly part of the reason but it was done more to gain a servant after his previous one was murdered to reveal Riders noble phantasm. that and Kirei had no real loyalty to anyone which is one of many reasons Kiritsugu was very cautious of him more than anyone else in the war.
I wouldn't say Emiya enhanced it as it seems more like a weaker version and Emiya isn't capable of replicating a full noble phantasm. the page says that Hrunting ends up acting more like a bullet especially when exceeding mach 10. thus, I think Emiya uses a weaker version of the tracking effect that instead of tracking a target it more like nudges itself to stay on target utilizing a form of normal bullet physics. (or rather strange arrow physics because it's still an arrow) well as normal as it can get without noticing it move itself through the air.

I don't mean to sound harsh as I know a lot of people like Jeanne Alter. (I, myself, love her Lily form) but the throne literally doesn't even think that Ruler and Avenger are the same person.
In the case of Medusa it seems the throne has separated out her existence into different classes based around different points in her life (this seems like a common thing for most heroes and such that don't utilize different fighting forms or weapons at one particular moment in their life.(note: this isn't a good way to describe it.) Gilgamesh seems to be similar to Medusa in this regard.). Ana is her form from when she was young and (possibly) not cursed by the gods. Rider is from around the middle of her life it seems as she still is deep down a decent person (more so than her sisters.) but it is evident her mind and morals are not exactly the innocence that her younger self is. Gorgon or Avenger is taken from the moments leading up to as well as the moment before Perseus kills her.
So @Swiftly Asian isn't wrong for say it's just way more complicated. in short the throne separated the spirits into the classes according to the requirements of the grail and thus kinda just sees them as copies that differ one to another.
Not really Astolfo lacks sense, he never really went mad as far as what is known about him. (according to what I have seen on the wiki) because I am actually not familiar with his legend I can't say for sure.
I kind of forgot to say that I can't speak for the Admin's though. it is certainly something you could ask them.
The Nasuverse is large. a few hundred or so editors can't be expected to find the information about a certain aspect of something and then organize it at the same time. which is why the wiki is still looking for editors. it's quite clear these pages aren't perfect. (I, myself, spend time looking for Grammar errors and fixing them a lot) so honestly, if you feel you have the time, the wiki could use someone to organize pages if possible as well as other editing tasks.
F/go is also still expanding so maintaining information becomes an issue for this page too.
There is a possibility that you could get a heroic spirit (I.E. Saber) to murder you. it would still suck just as bad though.

This is a hard question without proof. I am currently searching for proof that one can only summon one specific version of a servant though I'm positive it's just common knowledge. I did find the following however:
As beings outside of time, they can be summoned to any era regardless of the notions of past and future, even if summoned to when their human selves have yet to die or when they have yet to be born.

I do suggest reading the servant page too but I have searched that page to answer the above question and haven't seen anything. ( I might have missed something though.)
The best answer that I can say is that while Ana and Gorgon are the same person, the corrective impulses of the world doesn't see them as the same person mostly. it helps that their personalities are different too.
The way I see it, you can only summon one version of a given servant but you can summon alternate personalities or even younger versions of the servant because it's highly possible that their personalities are different enough that you really can't call them the same person. also Servants are only copies of the actual Heroic spirit or whatever.
Also FGO can get away with that kind of stuff because in it's world the history of man is thrown into disarray and it can't tell what is supposed to exist. (also why Kiritsugu Emiya is capable of being summoned despite not existing on the throne of heroes) it's page is here.

Technically the answer you're looking for but I also know that it isn't. the above is true until you hit the golden rule of Fate. "there is an exception to everything."
Truth be told you have to realize that how people see past Hero's or whatever may not necessarily be how they actually are. basically there are Berserker's that Mad Enhancement doesn't effect (ex. Beowulf).
So the actual answer you're looking for is "it depends on who you summon." also what Pkola54 said is also true.
I said he doesn't care because I needed to wrap up my thought, I am quite aware this isn't entirely accurate but Shirou can be hard to predict when it comes to things that aren't related to his ideals. so honestly it's a real struggle to figure out if he would actually try to fight Rin about his name or not.

Thanks man and I agree. I see the scene play out like this; sometime after the proposal Rin totally starts freaking out about this subject. Shirou then, after hearing the stresses Rin has about it (sucks man), tells her that he will forgo his name to appease her. at that point she will either stop stressing over it or it will get worse. (honestly this would be a pretty good comedic scene) in the event it gets worse, it will be an underlying stress for Rin till the last minute in which either Shirou or someone else will just force the Tohsaka name for the stated reasons that were discussed. Also because Shirou doesn't actually care.
